Addiction is a lot like cancer. Active it eats you alive, but in remission it is not affecting you as negatively.
Rehab can teach you the tools, but you have to be willing to learn and to use them. NA, AA or any other group of addicts helping addicts can also teach and help you know when and where to use the tools you get.
Support from friends and family is a big plus.
Putting and keeping addiction in remission is mostly up to the addicted. After all, they are the ones who have to make the choices, and when to make them.
Addiction doesn't have to be where one stays, Addicts do recover. Some for a lifetime some for years, some only for a short time. But always the addict comes back seeking once again what recovery has to offer. LIFE.
All it takes is a desire to not use anymore to find recovery. Try NA or another program...people there will listen and hear. They will understand.

In most cases the things that help you trough passing an addiction are the hospital or your family and the friends can help you to to pass an addiction. But now the help they give it to you depends too in what thype of addiction you are.. Good luck!
