My cousin has a 4 yr old who used to do this type of thing when he was a bit younger. He would head butt everything. One time, he actually smacked his head on the pavement, and started bleeding really bad.
My son hits and kicks the wall, chairs, etc.. but he doesn't head butt or whatever. He really dislikes the word no. He throws huge fits, etc.. I have spoken with a behavioral support person from his special preschool (he happens to have Autism) about his behavior. She suggested a "calming" area. It is basically like a place to have a time out, but it's padded with blankets, pillows, whatever. He has to stay there for long enough to calm himself down, and then he can do his self-destructive behavior w/out hurting himself. I keep my son in his calming area until the timer goes off, assuming he has calmed down. If not, he stays til he is fulling calmed.

When I've tried ignoring him to see if he'd get over it, after awhile he'd go lie in his room and sulk for awhile so lately as soon as he starts chucking one i'll pick him up and put him in his room until he's done.
hes not banging his head as much now, he turned to running into walls instead :( its actually looks pretty funny lol but not good

I also have a 2yr old son!!! he does the same thing!! I've just been ignoring him or tell him to hit his head harder and then he just looks at me like i'm crazy and stops!! lol since i've been doing that he's has stoped banging his head except when he really gets mad but i still just ignore him because he wants my attention and if he doesn't get it then he stops.

my almost 2 year old does the head banging thing also, i got realy woried but the doc said that she knows what she is doing and she understands pain and that she wont or shoulndt go to far is in realy hurting herself, but i agree with the other poster i got to a point that i ignor it and after a few minuets she will calm herself down and then ask for huggies.

Miss 2 doesn't bang her head, but she screams and tries to dislocate my fingers.. I just try and be consistant by saying no and if she conitues she is put on her bed for a rest.. Gives us both some time out and she comes out and is happy again..
