To Webmasters Or Programmers Only... Need Help In Adding Dropins

Hi guys,
I would like someone to help me learn how to add "dropins" or "drop downs" or whatever the h*** they are called to a site/blog.... The slide that drops down over a site when it is viewed, and offers freebies or info maybe and in return collects the visitor's info... not a pop up, but a drop in coded in java I think.....
I can't seem to find a site that provides it, or teaches how to implement it. I already have a code for one, but it isn't functioning...any help?

use FORM tag
WITHIN IT USE select TAG with attribute name ="name"
within the opening and closing SELECT TAG use option tags (for each option you want to have in drop down list you have to make opening and closing of option tag)
