taking a risk and moving up at work

I'm currently in a bit of a turmoil - I have worked for the same company for nearly 4 years, and have moved up from case worker, to acting team leader, to team leader... and now another opportunity has come up - to move up to a service manager position. The dilemma? I will have to pack up my family - hubby & two kids (9 & 12 y/o) and move 1000kms away from where I currently live... it's a great opportunity, with around $15k pay rise... and is a great career move for me... so why am I hesitating?
I will leave behind here my Mum and my sister & her family... my other two sisters & their families live another 1000kms in the other direction... so I'll be even further from them as well... I will leave behind the friends I have made here in the 4 years I've lived here.... my kids will have to change schools and leave their friends behind... hubby will have to leave his job & friends behind (he will get a job anywhere though)...
I'm feeling scared & uncertain.
Have any of you taken a risk & moved far away from your home to further your career?
