Doubt concerning Ben's surgery

I remember in the last episode of season 2, Ben let micheal leave the island with his son in a ship. If so, why don't he himself leave the island and had the surgery in the outside world? Instead he choose to risk his life and take his chances to let Jack to operate it with him. Does he has a death wish or what?

I can't figure that out either. I am wondering if there is some reason these "others" can't leave the island. They keep the whole thing such a mystery, it is hard to figure out what is going on sometimes. But then again, I suppose that is why we keep watching. It is so we can figure out that mystery.

I've one theory on mind: Ben already knows he's gonna die anyway(properly proved by several doctors already). The reason why he still wants Jack to do the operation is a test against him(?!)
You can see my other topics concerning Juliet, I think they're actually acting to fool Jack.
