July 21st, to be exact. This was announced on Rowling's website this morning. The US publisher is Scholastic Children's Books and the price will be $34.99 hardcover, $65.00 deluxe edition, and $39.99 for the library quality reinforced edition. The title has been confirmed as 'Harry Potter and the Deathly Hallows'.

The movie 5 (Harry Potter & the Order of the Phoenix will be shown on July 13th and the last book (7) of the series entitled Harry Potter & the Deathly Hallows will be publish on July 21st.
