how do you deal with a hot tempered child?

what my 4 yr. old son wants he gets. otherwise, he'll be shouting at his nanny or annoys everyone til you give what he wants. we tried to ignore him when he kept on shouting and he got a fever coz of sore throat and cough. i don't want that to happen anymore. he has a short temper indeed. moreso, his voice is high pitched so its normal for him to shout even if he's not angry but most of the time he is.
