I would recommend a hand held steam cleaner. That is what I used when my children were potty training and first started wearing underwear to bed. Once the stain is gone, the smell should be gone as well. I also would spray the mattress with FeBreeze in order to make sure that the mattress smelled fresh after the cleaning.

If none of the products that you tryed on the mattruss works I would say you have to buy a new mattress.I worked at a motel and there was a lot of urine on the mattress. We tryed a lot of different products, but when the mattress dryed the stain came right back. And the smell never goes away.

There is a product you can get at pet stores called "Natures Miracle" It works very well. You can buy it at most pet stores of order it online.
I have 5 kids and have been a foster parent for 8 years. I have dealt with a lot of bodily fluids!!!
I also use a Bissel "Little green cleaning machine" which it like a mini steam carpet cleaner.
Hope this helps.
