what really causes dandruff?

There is no commonly agreed upon theory about what actually causes dandruff. any number of reasons are possible both internal and external.
here are just a few possibilities:
poor health, excessive use of sprays and gells, hormonal inbalances, cold weather, dry indoor heating, stress, improper nutrition, heredity.

one thing about dandruff shampoos, you need to keep using them or the dandruff usually just comes back.
if your husband has tried many of the dandruff shampoos on the market with no change, then perhaps he should see his doctor about it. the doctor might be able to prescribe a prescription strength dandruff shampoo instead.

Have your husband tried Nizoral? He doesn't have to use it daily, and I find it quite effective. I usually develop dandruff when I'm stressed out. And after using it for about a week, my dandruff disappears.
Of course the effect may differ with your husband, as his lifestyle might be different from mine (i.e., what he eats, and/or his vices).

Diet plays an important role in the treatment of dandruff. To begin with, the patient should resort to an all-fruit diet for about five days and take three meals a day of juicy fruits.
Avoid citrus fruits, bananas, tinned fruits
Citrus fruits, bananas, dried, stewed, or tinned fruits should not be taken.
Well-balanced diet
After the all-fruit diet, the patient can gradually adopt a well-balanced diet, with emphasis on raw foods, especially fresh fruits and vegetables. Further short periods of an all-fruit diet for three days or so may be necessary at monthly intervals till the skin's condition improves.
Meats, sugar, white flour, strong tea or coffee, condiments, pickles, refined and processed foods should all be avoided.
