during DNA replication how are mutations formed?

i learned about it in biology but i still dont understand why diseases like cancer get started

small error in enzymes for proof reading will give dramatic effects and our food habits and environmentral effects can cause adverse effects like cancer

while the strand is being broken by the helicase the receptors change the nucleotide sequence resulting in mutations.
