What is the number one reason you have chosen to not return to a restaurant?

I could give a few, but the number one would be rudeness. We have a new uppity restaurant in our area and they really try hard to be hoity toity. lol.....nobody is impressed except for those who feel the need to impress. They do not start serving food until 5 PM and they cannot tell you the price for their specials one minute before 5 PM. They say they have no idea. On top of that if you are one of the first ones there they will try to seat you at this tiny two seater table built for those who starve themselves and if you ask for a better table, they sigh.

that would be my number one reason too - lack of cleanliness. although we would never really know what could have they done to our food before it was served to us, i still resent a restaurant who is obviously untidy and dirty.

Rudeness that's for sure. I live in a small town, population 35 thousand, so end result our choices are somewhat slim. And there were times that we preferred to rather stay in as to go out, and start hunting for our food. That is what I call it. Because you start at a particular restaurant and if it is not satisfactory you move on to the next, and the next.. That's why I call it hunting.
Here is a true story that happened to us, it was even in our local newspaper. A waitress at a particular restaurant, treated us so badly that my husband wanted to see the manager, which she replied, she is the manager, then I stood up and went to reception, there I asked who the owner was, they told me, it is the waitress who served us. We could not believe or ears or eyes, nevertheless we decided not to eat there but to finish our drinks. My husband gave a 2 cent tip, and wrote on the the bill "Here is 2 cents to phone lifeline and get a life"
We got up and as we walked to the door here comes the 2 cents flying through the air inches from my head.
They had to constrain my husband, he was so mad. The next day it was in the newspaper, with the heading, customer almost beheaded in restaurant. For weeks people commented on that, and told how they got treated at the same restaurant. She closed down about 3 months later. Thank God.
Now we laugh at this but at the time it was not funny at all:)

I really hate it when we get lousy service at a restaurant, but cleanliness is much more important to me than service. One time my sister and I went into a restaurant in a nearby town. We had gotten our drinks, but not our food yet when a cockroach ran across our table. Needless to say, we did not stay for our meal and have not returned to that restaurant since.

That's a tough call. I suppose my number one reason would be if they really didn't have any food I liked. A very close second would be if the restaurant smelled too smoky to the point where you can smell the smoke in the non smoking areas. Smoke bothers my allergies and asthma so I really can't tolerate an eating atmosphere like that.
