anatomy and physiology of a kidney

actually, besides the complete image of the anatomical and physiological view of a kidney or renal system, i am also looking orward if you can give me an image too of the pathophysiological view of a chronic renal failue. Also, can you give me an explanation on this: "anemia secondary to chronic renal failure secondary to chronic glomerulonephritis secondary to hypertension"---what do you mean by that? i only need the simplest form of discussion about this matter, please and thank you very much...
