Hi, my friend.
I am working on a translation task recently.
A sentence from it I can not understand. Would you give me a hand and have a look at that?
More words are given as context. What I do not understand is marked as CPTITAL WORDS.
Here it is:

Bad communication is probably the biggest single cause of complaints and it is often closely linked to the topic of honesty. Unlike previous generations, todays' doctors have had extensive innstruction in commnunication skills but BUT IT IS A FEATURE OF THE SYSTEM THAT IT IS OFTEN THE GREENEST SHOOTS WHO ARE THE MOST EXPOSED WHETHER IN A AND E OR AS THE HOUSE OFFICER CLERKING IN A PATIENT.There is a world of difference between acting out how you wold deal with a particular situatino in a workshop and having to face the cold reality of telling a patient that he has cancer.

I know that house officer refers to the intern doctor and A and E means accidence and emergency department of the hospital. But what is meaning of all the captical words above?

...from what i understand, it's most likely like this:
...but it is a regular "feature" that those who are still green (new) are the most exposed (to the problem) whether in A & E, or the medical officer checking in a patient
I think the writer is comparing the inability of newbies in communicating compared to the older generations, and those who are in this category from the A & E department or medical doctors who handle patients who are checking in.
Could "feature" be "failure"?
I assume that "clerking" was supposed to mean "checking" based on context.
Does it makes sense? :)

Well my dear friend it means that the new and inexperienced house officers are exposed to the patiences and these guys with bad communication will always bring trouble to the hospital management.. This is what I understood from that line. All the best..
