This is a difficult situation. Hope this will never happen to me in real life. I am not a very courageous person to face this. I will better try to run away in this case. I don't usually use public restrooms in malls and trians because usually they won't be clean and to aviod attacks from strangers. So, I am confident that the above mentioned situation will not happen to me.

The footsteps you heard would be the aunty who stands outside the door barring anyone from runnig away from the fire exit. :) heehee

heehee.
1. I would check if there is a fire exit and try to squeeze out.
2. I would go out and tell him/her that I am also a serial killer and exchange notes on modus operandi, frequency, methods etc.
3. I would lie low, hide behind the closet
4. Just act normal and ignore (after all how do i know s/he is a serial killer)
5. Act dumb and ask ho wmany "serials" s/he has killed so far?
