There are basically three reasons for crying - physical pain, joy, sorrow. However, no one knows if the tears that you see in a person's eyes is a true reflection of her condition or emotion. People could pretend to cry by thinking of something sad - that happens in the movies often.
There are other reasons why eyes secrete tears. Tears come out from your girlfriend's or boyfriend's eyes after they sneeze - so it doesn't mean they're crying due to sadness. Your mom may have tears in her eyes but that's probably because she was peeling onions.
What actually causes tears to appear has something to do with the brain and the glands in the eyes that produce tears. When you feel happy or sad, there will be a chemical reaction and the brain tells your eyes to "cry". I'm not good in biology, but I believe it works that way. But correct me if I'm wrong; after all, this is a place to learn :)

I don't think so, it is possible to not cry when you are extremely sad, and it is possible to cry when you are not sad at all.
Crying can be as much a sign of someone having dust in their eye, having just yawned or sneezed, or having just looked at a light, as it can be a result of sadness.
