Standard Definition (SD) to High Definition (HD) - How is it done?

I'm a big fan of HD material, and if something is in HD rather than SD, I would go out of my way to ensure I watch the HD version.... quality is everything to me.
I've heard of old TV shows and movies being upgraded so to speak from SD to HD. I was wondering how this was done? Surely they were not filmed in HD to begin with right? An example is the original series of Star Trek, season one is out on BluRay in HD but how is this possible? What's involved?

It is true that today movies also offer HD versions. I am not sure whether it is the same for TV shows. Since our TV set is old, so when we play the games by Xbox 360. Sometimes I hardly see the words clearly because I need the HD TV set so that I can see it.

There are many TV shows that are available in HD but what I don't understand is how old TV shows like the Original Star Trek made in the mid 60's can be re-released in HD.
