I'm American, so you've already nailed what we say! It's usually just how are you? and then fine, how are you? Because we're lazy and really don't want to spend the time elaborating on that. :P
I work with a lot of Mexicans, though, and everyone likes to speak as much Spanish to them as possible, so when they ask Como estas? (How are you) we answer with something like Estoy bien (I'm good) or, more often nowadays, estoy cansada (I'm tired). ^_^
And I see someone mentioned french earlier, so I'll just throw out that in French it's comment ca va? (How are you?) and the response is ca va (fine) or ca va bien (I'm good).

For Mandarin,
ni hao ma? - How are you?
wo hai hao? - I'm fine.

We Filipinos say "How are you like" this: Kumusta ka? Then we say "I am fine" like this: Mabuti naman ako.
