Well, I'm online most of the day so I can just look up the meaning in the online dictionary I'm using. But if the computer is rather far, I will just have to continue reading and put the word in the overall context to understand the author.

If I really have hard time understanding the word I would rather go for thesaurus cause I could find the words and then at the same time it will add to my vocabulary if I will get the synonyms and antonyms of those words.

this is possible only with few words and not all,
this is guessing and guessing can be correct or even wrong.

Usually I try to work it out from the context. It's likely that the author will use the word again a few paragraphs or pages later, and from that extra information I'll be able to guess at a meaning. If I stumble across the same word again and it's within a context that would call for a completely different meaning... then I look it up and make sure I have it right. If a word I don't know just strikes me as interesting, I'll probably look it up the first time.
