Mylotters have given you sevral answers. But if you ask a child, I`m sure he also knows how to explain that he is hungry. He might not knoe (or care) about the biological reasons behind it, but hunger pains are still there.

Sorry and thanks for asking. The terms "gutom" (hungry) and "kay oga" (hazy local term applicable as a reply to all questions). It's important for a child to have a working knowledge why he feels hungry. What's happening inside his stomach? It's for us parents to explain to them the process. It's sad to say that some elders can't give a satisfactory answer or explanation to their offspring on the issue.

according to the merriam webster medical dictionary, hunger pangs are related to the contractions of our empty stomach and/or intestines

We say in hindi phrase when feel hunger means "Rats dancing in stomach" haha...funny but fact when feeling hunger means like someone dancing in it.

Children are active and the food they intake is digested that fast. It is the metabolism that makes them burn the food that fast. You can ask, is it painful? If the child cries, it is a sign that a child is either hungry or in pain.
