Oh, wow! I've gotten letters with those phrases used by the city government here several times.
When we first moved into our last house, over 22 years ago, our second water bill was over $500, up from $13, for a month. Then the next month it went back to $13. The city water department kept giving me the runaround until I finally threatened to sue the city over it! Right after that, I got a letter that sort of apologized for the inconvenience of a "mistaken" reading on the water meter. *shake my head*
