Untrained staff, wrong orders, cold meal after waiting and waiting, wrong stuff on the table even the waiter have repeated the order till he annoyed you, and you have to ask about everything, glass of wine served to after the whole kitchen staff signed with their fingerprints on it (yach),
At the end you may get billed for stuff never ordered

Service in the US tends to be pretty good. Clean sanitary conditions and usually good service unless people call off sick.
Most drinks come directly from the bar her touched only by the bartender and your waitress.
You do have to check the bill if you're with a large party but usually if I'm by myself it's right on the money.
Occasionally you get a waitress who has had a bad day but as long at she's apologetic and appears to be trying I don't penalize her in the tip.

I think alot of it stems from where they started calling it "fast food". Plus everything is so fast paced now. they whip that food up as fast as they can and send it out the door as fast as they can. I noticed that alot of restaurants hire a bunch of young kids to do the work.
