This is a quote from Prevention's website.
An overabundance of odor-producing bacteria in your mouth, particularly on the back of your tongue, is the most common cause of bad breath. A bit of self-help--including a dedicated regimen of tooth and tongue brushing--will usually do the trick, explains Richard H. Price, DMD, a professor at Boston University Dental School.
There are other less-common causes, however. Dieting can contribute to bad breath, as can postnasal drip. In more serious cases, sour breath may be a symptom of health problems--including respiratory infection, gastroesophageal reflux, uncontrolled diabetes, and kidney failure--that require medical attention, says Mahvash Navazesh, DMD, chairman of the section of oral medicine and oral diagnosis at the University of Southern California School of Dentistry in Los Angeles.
When It's Something Serious
A whiff of your breath can help your doctor determine whether your problem is physical rather than just social.
Here's what certain types of bad breath can mean:
Sweet and fruity--Diabetes
Ammonia/urine--Kidney failure
Rotten eggs--Cirrhosis of the liver

well the story of halitosis is not new,but the causative factors should be the one everybody need to be familiar with.To a large extent several conditions pertaining to the oral cavity can cause bad breath.Accumilation of plaque, calculus(mineralized plaque).Presence of various disease conditions such as
gingivitis, periodontitis, various abscessess in the oral cavity.pericorinitise.t.c.
Apart from the factors in oral cavity various systemic factors like sinusitis, post natal drip, starvation, use of various medications, certain digestive problems all can lead to halitosis

I do believe it is caused by post nasal drip, or something to do with blockage in you nasal cavity or something. Like when the mucus gets stuck in there it probably decays or whatever causing that awful smell.
I think nasal irrigation can help keep bad breath away. =)

it really depends on the person. some people create it on their own, and others have it because of bad hygiene (not flossing or brushing). i have heard though that a natural mint leaf does wonders, and baking soda is really good also
