Simply bring at least two valid identification cards, passport ID picture and NSO birth certificate ..
If there is nothing wrong with the entries in your birth certificate, then the above reuqirements will suffice.
You have to pay P750 for 15 day processing or P500 for a 30-day processing.
Good luck.

You have to secure your Original Birth certificate from the NSO, passport size id picture white background with a colar shirt, just prepare also your baptismal certificate because last 2007 the agency where I applied my passport they asked for that. the cost depends in your application not more than 2000 including your fare and food as you process.

is it your first time to get a passport or just renewing? definately NBI Clearance is needed, the fee is - for regular process P 950 (20 working days excluding weekend), for rush process P 1,200 (10 working days excluding weekend), FYI: other documents/requirements, birth certificate (NSO), photocopy or any valid ids like PRC, drivers lic., sss id, tin id or school id, marriage certificate (NSO)-if you are married, voter's registration record (first timer)..you can also go to this site for furthr inquiry http://www.passport.com.ph/what-to-expect - hope this helps

Hello vycess, there's no need to bring your NBI all you need to bring is your original birth certificate from NSO. Else you can apply online so you can get your priority number ahead before going to DFA. www.dfa.gov.ph for e-passport application.
For the costing, it depend if you want to have it for 14 days or 24 days. The longer the duration it is cheaper like 950pesos I think and 14 days will cost I think around 1,200.
Goodluck to you and Happy mylotting.
